摘要 近年来,颈髓损伤呈明显上升趋势,多由外伤引起颈椎骨折,压迫脊髓腔内血管神经,引起水肿、损坏.上颈段脊髓损伤(C1-4) 此段脊髓上端与延髓相连,故损伤后部分病人可合并有延髓甚至脑干损伤的临床表现.上颈髓损伤时,常有颈枕部疼痛,颈部运动受限.c1～2损伤时病人大多立即死亡,C2～4节段内有膈神经中枢,伤后多出现膈肌和其他呼吸肌麻痹,病人表现有进行性呼吸困难,损伤平面以下四肢上运动神经元性不完全瘫痪.下颈段脊髓损伤(C5-8) 此段损伤多引起肋间神经麻痹,膈肌麻痹,四肢瘫痪,双上肢为弛缓性瘫痪,双下肢为痉挛性瘫痪,损伤平面以下感觉丧失,C8～T1损伤可出现尺神经麻痹的爪形手和交感神经节受损的Horner征.该病具有极高的致死率和致残率.因此,对颈椎损伤必须予以高度重视.颈髓损伤后各阶段都有可能出现并发症也是导致患者死亡的重要因素.损伤早期并发症的护理、围手术期的护理、术后康复护理对提高救治成功率和降低致残率有着极其重要的意义.

Abstract In recent years,spinal cord injury showed an upward trend,multi-trauma caused by cervical spine fractures,spinal cord compression cavity vessels and nerves,cause edema,or damage.On the cervical spinal cord injury (C1-4) this block of the spinal cord and medulla connected to the top,the injury after some patients may be associated with brain stem injury in the medullary even clinical manifestations.On the spinal cord injury,often Jingzhen pain,neck movement restricted.c1 2 injury most of the patients died immediately,C2 4 segments with phrenic nerve center,and more injured after a diaphragm and other respiratory muscle paralysis,patients have the sexual performance of breathing difficulties,damage on the plane following the limbs of motor neuron Is not completely paralyzed.The cervical spinal cord injury (C5-8) this section is more damage caused intercostal nerve paralysis,diaphragm weakness,paralysis of limbs,upper limbs for the flaccid paralysis,both lower extremities for the spastic paralysis,injury plane following the loss of feeling,C8 T1 Damage to the ulnar nerve paralysis in the feet and hands-damage to the sympathetic ganglion Horner levy.The disease has a high rate of death and disability.Therefore,cervical injury must be highly valued.After all stages of spinal cord injury may also lead to complications in patients with the death of important factors.Injury early complications of care,perioperative care,postoperative care and rehabilitation to improve treatment success rate and reduce the morbidity is extremely important.
