Hereby affirm that I/we have been advice and informed thee inherent hazards of scuba diving.I/we understand that diving with compressed air involved certain inherent risk; decompression; sickness; embolism; or other hyperbaric inherent can occur that require treatment in a recompression chamber.I/we still chose to preceed which such dive in spite of the possible absent a recompression to the dive side.
I/we understand and agree that neigher my instructor the facility through which I/we receive my instruction nor international PADI,inc.nor any of their respective employees,officers,agent or assigns.(hereinafter referred to as ‘released’) maybe held liable or responsible in any way for any injury,death or other damages to me or my family heirs or assigns that my occur as a result of my participation in this diving program or as a result of a participant in this program,including all risk connected therewith,whether foreseen or unforeseen.
The purpose of this medical questioner is to find out if you should be examined by your doctor by your diving.a positive response means that there is a preexisting condition that may effect your safety while diving and you may seek the advice of your physician.
if any of this item apply to you we must request that you must consult your physician prior to participating in scuba diving.Your instructor will supply you with PADI Medical Statement and Guidelines Recreational Scuba Divers Physical Examination to take your physician.

IT IS THE INTENTION OF THIS INSTRUMENT TO EXEMPT AND RELEASE MY INSTRUCTOR(S) FROM THE RED CORAL THROUGH WHICH I/WE RECEIVE MY INSTRUCTION FROM INTERNATIONAL PADI INC.ALL RELATED ENTATIES AS DEFINED FROM ALL LIABILITY OR RESPONSIBILITY WHAT SO EVER FOR PERSONAL INJURY PROPERTY DAMAGE OR WRONGFULL DEATH HOWEVER CAUSED INCLUDING ALL PROCEDURE BUT NOT LIMITED TO THE NEGLIGENCE OF THE RELEASED PARTIES WHETHER PASSIVE OR ACTIVE.
